4. **Unit 4: Contributing to the Care of a Person with a Long-Term Condition**
   - Understand long-term health conditions and their impact.
   - Assist individuals in managing their conditions.
   - Promote self-care and independence.

5. **Unit 5: Facilitating Person-Centered Assessment**
   - Conduct holistic assessments of individuals' needs.
   - Create care plans based on assessment findings.
   - Collaborate with multidisciplinary teams for comprehensive care.

6. **Unit 6: Building Relationships in Health and Social Care**
   - Develop effective communication skills.
   - Establish rapport and trust with individuals and their families.

7. **Unit 7: Safeguarding and Protecting Individuals in Health and Social Care Settings**
   - Recognize signs of abuse and neglect.

8. **Unit 8: Promoting Health and Well-being through Physical Activity**

9. **Unit 9: Supporting Individuals with Dementia**
   - Gain insights into dementia and its different forms.

10. **Unit 10: End of Life Care**
    - Explore the principles and practices of end-of-life care.
    - Support individuals and families through the end-of-life journey.

Assessment is via assignment submission

The fee for the programme is as follows:

• 6 months - Accelerated mode @ GBP £1250
• 9 months - Standard mode @ GBP £950

The programme offers following fee payment plans:

6 months programme
● Payment option (a): GBP £416 x 3 monthly instalments
● Payment option (b): GBP £1187.50 x 1 instalment (We offer 5% bursary on total fee for students opting to pay in full)

9 months programme
● Payment option (c): GBP £190 x 5 monthly instalments
● Payment option (d): GBP £475 x 2 quarterly instalments
